This Consumer Health Data Privacy Policy describes how Maltby Manor Senior Living collects, uses, discloses, and protects consumer health data in accordance with applicable Washington law, including Washington’s My Health My Data Act (RCW 19.373).

As a state-licensed adult family home owned and operated by a Registered Nurse, we are committed to maintaining the utmost discretion, integrity, and security across all physical and electronic health records entrusted to our care.

Depending upon the information you voluntarily provide to us through our contact portal, direct telephone consultations, medical records transmissions, or pre-admission assessments, consumer health data may include:

Sources of Consumer Health Data

Consumer health data may be provided directly or authorized by:

The prospective resident directly
Current residents
Family members and designated caregivers
Authorized legal representatives (e.g., power of attorney or legal guardian)
Authorized healthcare providers, case managers, or hospital discharge planners
Other authorized individuals communicating on the individual's behalf

Sharing & Disclosure Restrictions

Maltby Manor may share consumer health data only when reasonably necessary, strictly consented to, or permitted by applicable Washington and federal law, including with:

Authorized Maltby Manor licensed and direct-care personnel
Treating physicians, nurse practitioners, and healthcare providers
Dispensing pharmacies and pharmaceutical consultants
Hospice and palliative care providers
Physical, occupational, and speech therapy specialists
Emergency medical services (EMS) and emergency hospital staff
Authorized resident family contacts and designated legal representatives
Washington State Department of Social and Health Services (DSHS) and regulatory bodies when legally mandated
Trusted professional service vendors bound by written privacy, HIPAA, and data protection contracts

Absolute Prohibition on Data Selling

Maltby Manor does not sell consumer health data. We never trade, rent, broker, or monetize any health information or contact details with third-party advertisers or commercial entities.
